While quite a few of the items on your list are spot on my bad habits as well, I have to ask you why you are still working for the same company. Since i left my previous employer, bi-weekly migraines, chestpains, shoulder and lower-back hurts have magically disappeared. ( I realized at the end of the 2nd year there, that the environment/workload wasn't good for my health, but i still stuck with them for 5 years in total).
There could be (and probably are) very good reasons why you're still with them, but i couldn't help but wonder.
