the module name for ithreads is "threads":

perl -e 'use threads; print "ok\n"'

As for the OP, in my ithreaded perl both "usethreads" & "useithreads" (and "usemultiplicity") are set. Maybe you need -Dusethreads and -Duseithreads (and -Dusemultiplicity) to compile perl with ithreads - I don't know for sure since I always do a "manual" interactive Configure.
