Read HowTo build and distribute a PPMed module for Win32. All I do is drop a bunch of ppds and their associated ppms in a directory, and let apache dump an index.
The other thing you could do, which I personally haven't, is run a PPM server ( http://search.cpan.org/src/MURRAY/PPM-2.1.6/server/ ). I took a look at it once long ago and decided it wasn't for me (when ppm3 is released on cpan, maybe i'll consider it -- for now, i'm content with a HTTP ppm repository).
