I think you need to actually change the text in the label. This should make it refresh. I would suggest looking at this. When I did VB applications this is what we had to do so I think it would be the same for Perl. This has not been tested and it is just my weekend ramblings.
#!/usr/bin/perl
use strict;
use Tk;
my $top = new MainWindow;
my $not_stopped = 1;
my $id = "";
while($not_stopped){
$id = $top->Label(-text =>&gettime);
sleep(10);
}

Posts may use any of the Perl Monks Approved HTML tags. Currently these include the following:
<code> <a> <b> <big>
<blockquote> <br /> <dd>
<dl> <dt> <em> <font>
<h1> <h2> <h3> <h4>
<h5> <h6> <hr /> <i>
<li> <nbsp> <ol> <p>
<small> <strike> <strong>
  <table>
<td> <th> <tr> <tt>
<u> <ul>
-
Snippets of code should be wrapped in
<code> tags not
<pre> tags. In fact, <pre>
tags should generally be avoided. If they must
be used, extreme care should be
taken to ensure that their contents do not
have long lines (<70 chars), in order to prevent
horizontal scrolling (and possible janitor
intervention).
