As I said in this node :

IIS doesn't like relative paths!
This would work for you:

($relative_path=$ENV{SCRIPT_NAME}) =~ s|[^/]+$||; $absolute_path=$ENV{PATH_TRANSLATED}.$relative_path; $myfile=$absolute_path.'myfile.tmpl'; &Template($myfile,\%form);
