Why not just make it like a balot (voting) paper with a box the user fills in with their numerical choice next to each statement. This is a simple paridigm.

If you want to get fancier you could use an onBlur() javascript to validate that the user only uses each number from 1 to 20 once. Alternatively the numerical votes could come from a javascript function that provides a drop down list of the possible votes. As a number is used it could be removed from the available votes drop down list. If you want to go further you could rearrange the list in real time as choices are made.

Snippets of code should be wrapped in <code> tags not <pre> tags. In fact, <pre> tags should generally be avoided. If they must be used, extreme care should be taken to ensure that their contents do not have long lines (<70 chars), in order to prevent horizontal scrolling (and possible janitor intervention).
