This is a question I ask all my programming candidates: "Tell me about the worst bug you ever wrote."

Then, after he/she tells me the story, I ask "What did you change in how you work as a results?"

The questions, and their answers, dig at the person's depth of experience, and maturity as a programmer, respectively.
