my $instance = 3;  # 3rd

scalar $str =~ /\G.*?$regexp/gc
   while --$instance;

$str =~ s/\G(.*?)$regexp/$1$replace/;

print("$str\n");

The /c is needed in case there are less than $instance instances of the search string.
