Can you show the code you have so far?
If you are using CGI.pm then the hidden fields are available through calls to the param method, just as the file upload FH is. So before writing the contents of the upload, get the values from the param method.

<code> tags not
<pre> tags. In fact, <pre>
tags should generally be avoided. If they must
be used, extreme care should be
taken to ensure that their contents do not
have long lines (<70 chars), in order to prevent
horizontal scrolling (and possible janitor
intervention).
